Job Description
Saint Luke's Hospital in Kansas City has an opportunity for a Heart Failure Educator. THis position provides individualized heart failure education to patients admitted with diagnosis of heart failure and their appropriate care giver(s) as a supplement to the education provided by the bedside
nurse and interdisciplinary team.
Hours: 8 am -4:30 pm 2 days per week. No weekends.
+ Emphasize the importance of self-management and monitoring through education provided.
+ Assist with care coordination of inpatients by providing feedback to the treatment team, RN and care progression team of any obstacles to successful transition and follow-up identified during education session.
+ Utilize available resources, tools, and referrals for patients who are identified to possibly benefit from additional supports after discharge.
+ Assist program manager with planning and executing support group meetings.

Applicable Experience:
1 year
Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)
Bachelor's Degree - Nursing
