    About the Role

     

    Under the direction of the Director of Professional Services, the Coordinator of Rehabilitation Services will plan and implement prescribed therapy programs to help individuals maintain and/or restore function due to acute or chronic disabilities. This role includes conducting evaluations, developing treatment plans, completing documentation, and collaborating with multidisciplinary teams.

     

    You’ll play an important part in Richmond’s mission to enhance the quality of life for our residents, actively participating in quality improvement initiatives and ensuring the highest standards of care.

    Key Responsibilities

    + Review physician referrals, medical records, and maintain up-to-date prescriptions in compliance with NYS Department of Health guidelines.

    + Evaluate and assess needs in areas such as cognitive/perceptual skills, motor function, musculoskeletal status, adaptive equipment, and assistive technology.

    + Develop and implement individualized treatment plans, including therapeutic modalities and activities of daily living.

    + Maintain detailed, outcome-oriented documentation of treatment, progress, and recommendations.

    + Provide training to families and support staff on follow-through programs.

    + Collaborate with physicians, health care practitioners, and interdisciplinary teams on care planning and discharge.

    + Supervise Certified Occupational Therapy Assistants (COTAs) and therapists on limited permits, ensuring timely co-signatures and oversight.

    + Participate in annual and semi-annual care conferences.

    + Assist with staff training, development, and compliance with clinical documentation standards.

    Schedule

    Full-time with flexibility for some evenings, early mornings, and/or weekends. Local travel to Richmond’s group homes required.

    Qualifications

    + Bachelor’s degree in Occupational Therapy required; Master’s degree or higher preferred.

    + Current New York State Occupational license.

    + Minimum 2 years of related clinical experience.

    + Expertise in OT evaluation and treatment, therapeutic modalities, and adaptive/assistive equipment.

    + Valid driver’s license and reliable transportation required.

    Physical Requirements

    + Ability to lift up to 60 lbs. and perform occasional standing, walking, bending, and reaching.

    + Sufficient vision, hearing, and manual dexterity for documentation and patient care activities.
