Job Description
• A client in the Deltona, FL area is looking for a Case Manager to join their team. The position will assist with reduction in overall admissions, readmissions, and ER visits. The CM is responsible for successfully supporting patients with high-risk health conditions to navigate the healthcare system. The CM assists in developing patient empowerment by acting as an educator, resource, and advocates for patients and their families to ensure a maximum quality of life. The CM interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, social workers, and other educators.

Skills and Requirements
• Registered Nurse license (FL) and BLS certification
• 2 years of experience as a Care Manager
• Ability to interact and verbally communicate effectively with patients and all levels of personnel in a professional manner. • Medicare Advantaged Care experience
• IV certified
