    Responsibilities

    Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.

     

    + Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.

    + Recommend formulary alternatives when appropriate.

    + Compound and dispense medications for accurate delivery to patients.

    + Provide drug information and education to medical staff and patients.

    + Communicate with physicians and other clinicians as necessary to facilltate care.

    + Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.

    + Assist with academic functions such as serving as a preceptor for pharmacy students.

    + May float to provide coverage of pharmacy needs.

    + Provide patient counseling when required.

    Requirements

    Licensure/Registration/Licensure:

    + Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.

    Education:

    + Bachelor's or advanced degree in Pharmacy required.

     

    Additional Preferences

     

    Doctor of Pharmacy

     

    Completion of a PGY1 Pharmacy Residency with ambulatory care pharmacy experience; OR

     

    PGY2 Ambulatory Care Pharmacy Residency OR 3-5 years of clinical pharmacy experience in the ambulatory care setting without a pharmacy residency

    ADDITIONAL RESPONSIBILITIES:

    Under the direction of the Ascension Wisconsin Director, Ambulatory Care Pharmacy

     

    Services, the pharmacist ensures optimum patient outcomes related to medication use by

     

    preventing or resolving drug-related issues, optimizing the drug therapy of patients, and

     

    coordinating care across the health system.

     

    + Provides drug information to patients and families,physicians, nurses, and other healthcare professionals. Integrally involved in education ofpharmacists, residents and students.

    + Strives to increase knowledge through research andwriting activities. Using independent decision making and critical thinking skills, the pharmacistpromotes medication use that is safe, effective and considers the costs and benefits of therapyto maximize value.

    + The pharmacist interacts with other members of the healthcare team andwith patients to proactively address medication therapy issues.

    + 1.Performs disease state management and comprehensive medication review for patients in theambulatory setting.2. Critically evaluates disease state and adjusts medication therapy to optimize medicationregimens and improve clinical outcomes.3. Ensures utilization of appropriate and cost-effective medication therapy.4. Areas of focus may include, but are not limited to, disease state management (e.g.anticoagulation, diabetes, hypertension, hyperlipidemia, asthma/COPS), medicationmanagement, and adverse drug event prevention and management.5. Consults with medical providers regarding optimal drug therapy management to improve clinicaloutcomes and promote patient safety related to medications.6. Completes thorough patient assessments and applies principles of evidence-based medicine toestablish patient-specific therapeutic drug regimens.7. Provides essential medication education to patients to promote engagement inself-management activities, medication adherence, and safety.8. Conducts patient-centered clinical pharmacy functions and appropriately documents all patientcare activities in the electronic medical record.9. Demonstrates accurate and thorough documentation skills. Facilitates pharmacist-to-providercommunication of clinical interventions and recommendations.10. Adheres to clinical practices approved by the Pharmacy and Therapeutics Committee (e.g.collaborative practice agreements, clinical pharmacy protocols).11. Acts as a network resource and drug information expert. Provides detailed drug information andeducation to the health care team, pharmacy, patients, and community. Actively participates indepartment committees and effectively communicates with stakeholders.Develops, contributes, and applies evidence-based medication guidelines and/or protocols,medication use evaluations, and disease-based evaluations. Collects and analyzesmedication-use data per predefined criteria and formulates appropriate recommendations forimplementation.13. Precepts PGY1 and PGY2-Pharmacy residents and pharmacy students. Meets preceptor criteriaestablished by American Society of Health-System Pharmacists (ASHP). Contributes to staffdevelopment by assisting with training of new team members, including teaching andexemplifying clinical and operational skills.14. Performs other duties as assigned
