Anticoagulation Pharmacists are trained to help people manage warfarin (Coumadin) and at times other anticoagulation medications if needed, such as enoxaparin (Lovenox). They will work closely with other caregivers to measure the level of your blood-thinning medication and adjust medication if needed. After visits for at least 3 months, they will help to determine if a home meter might be appropriate for patients to measure the level of their blood-thinning medication. They will also be able to help coordinate plans when needing to stop warfarin for a procedure.
A caregiver in this position will work 10-hour shifts, 7:30am to 5:30pm. Mondays will be held at the X Building at Main Campus and Fridays will be held at Beachwood Family Health and Surgery Center. Weekend expectations on call will be 2 full weekends a year (from Friday to Sunday) remotely. Only 1 on-call requirement for holiday expectations.
A caregiver who excels in this role will:
+ Provide safe, efficient, patient-centered, evidence-based medication services.
+ Verify medication orders and/or dispense medications accurately according to state and federal regulations.
+ Document services in a complete, accurate and timely manner.
+ Ensure all pharmacy operations comply with national accreditation standards.
+ Maintain professional interactions with patients to facilitate successful patient experiences.
+ Educate and train students, residents, other caregivers, and/or patients.
Minimum qualifications for the ideal future caregiver include:
+ PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy
+ Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
+ Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days (for individuals required to administer immunizations)
+ Current Advanced Cardiovascular Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association (AHA) or American Red Cross. For caregivers required to respond to adult or pediatric cardiopulmonary arrest, this is required within one year of hire
Preferred qualifications for the ideal future caregiver include:
+ Board certification
+ Completion of a post-graduate year 1 residency (PGY1), fellowship or 2 years of relevant post-graduate pharmacy experience
+ Post-graduate year 2 residency (PGY2) or relevant experience for Clinical Pharmacists hired in a specialized pharmacy practice area (i.e. Critical Care, Oncology)
+ Prior experience in an outpatient ambulatory, or inpatient pharmacy setting involving anticoagulation

**Physical Requirements** :
+ Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
+ Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
+ Position often includes reaching, stretching, stooping, climbing steps and sitting.
+ Must have normal or corrected vision.
+ Must have ability to clearly communicate verbally by phone or in person

+ Follows standard precautions using personal protective equipment as required
