Sterling Park Senior Living (a 40 bed skilled nursing facility) has the opportunity to welcome a Social Services Designee/Care Coordinator. Our Social Services Designees/Care Coordinator's excel at serving, protecting and advocating for our residents and families. This role is essential for admission, care coordination, and discharge process in our community. This role works in conjuncture with another social services designee to care for our 40 residents. JOB SUMMARY:

Social Services Designees are responsible for the operations of the Social Services Department, providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential from admission to discharge. Assists in the education regarding aging, Resident Rights, facility services, etc. Participates and contributes as a member of interdepartmental teams (IDT, QAPI), following all established policies and procedures.

QUALIFICATIONS:

+ Post-Secondary education with a bachelor’s degree in Social Work and/or related field preferred.

+ Must be able to relate professionally, positively, and cooperatively with residents, families, and employees.

+ Must be capable of performing the Essential Functions of this job, with or without reasonable accommodation.
