    Job Overview

     

    This position is part of an integrated, interdisciplinary team (the TMIN Pediatric One Care Team) that supports pediatric patients, their families, and their pediatricians. Works with the TMIN Pediatric One Care team to help design, implement, and evaluate pediatric programs (e.g., asthma, developmental disabilities, obesity, behavioral health, etc.). Additionally, manages patient registries for each program to track the clinical care and outcomes of individuals with chronic diseases. Ensures the integration of registry use into clinical workflows, enhances data accuracy and reporting, and supports quality improvement initiatives. Collaborates with healthcare teams to improve care coordination and helps develop pediatric disease management strategies.

    Job Description

    Minimum Qualifications:

    1. Associate’s degree in health-related field.

    2. Three (3) years of clinical setting experience. This may include experience in a Nursing Unit, Care Management Department, Managed Care setting, PCP or SCP office setting.

    Preferred Qualifications:

    1. Bachelor’s degree in health-related field.

    2. Four (4) years of clinical setting experience. This may include experience in a Nursing Unit, Care Management Department, Managed Care setting, PCP or SCP office setting.

    3. Experience with program development and project management.

    4. Experience in training and supporting healthcare staff in registry utilization and workflow integration.

    5. Training or experience in pediatric disease management and preventive health.

    **Duties and Responsibilities:** The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

    1. Supports the development of new pediatric programs.

    2. Develops project plans to implement new programs; leads implementations; manages and escalates issues as needed to facilitate resolution; and assesses programs for improvement opportunities.

    3. Provides program and status updates on a regular basis.

    4. Manages and maintains pediatric registries, ensuring they remain up to date, relevant, and effectively utilized in clinical practice.

    5. Oversees data entry, ensures adherence to data definitions, and validates accuracy through periodic chart reviews and comparison with Electronic Health Record (EHR) data.

    6. Leverages registry data to identify gaps in care.

    7. Incorporates registry use into practice workflows, ensuring all team members understand its purpose and functionality.

    8. Provides training on registry use, data entry protocols, and best practices in chronic disease management.

    9. Ensures proper data backups and maintains registry security, aligning with regulatory and compliance standards.

    10. Works with internal and external stakeholders to enhance care coordination.

    11. Follows up with patients and support community outreach and patient education efforts related to pediatric disease management.

    12. Optimizes registry utilization and help improve outcomes.

    13. Generates and distributes reports to clinical teams, leadership, and external partners as needed.

    14. Assists with quality audits and help identify best practices.

    Physical Requirements:

    1. Occasionally lift and/or move up to 25 lbs (OSHA standard requirement).

    2. Primarily a sedentary role, but may involve walking, standing, and operating office equipment.

    3. Frequent communication via phone, email, and in-person meetings.

    Skills & Abilities:

    1. Proficiency in electronic health records (EHR) and clinical registries.

    2. Strong analytical skills to identify care gaps and variations in treatment outcomes.

    3. Strong communication, organizational, and problem-solving skills.

    •This role focuses on providing administrative and business support to the organization in order to achieve operational goals. In addition, this role focuses on performing the following Program Management duties: Program management, projects, and coordination of resources to meet accreditation standards, support strategic growth, and service line development. These positions typically support hospital-based programs. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An experienced level role that applies practical knowledge of job area typically obtained through advanced education and work experience. Works independently with general supervision, problems faced are difficult but typically not complex, and may influence others within the job area through explanation of facts, policies and practices.
