Chronic Care Management (CCM) Nurse We are seeking a compassionate and detail-oriented Chronic Care Management (CCM) Nurse to join our team. This role focuses on providing ongoing support to patients with chronic conditions, helping them manage their health, coordinate care, and improve quality of life. The ideal candidate will have strong clinical judgment, excellent communication skills, and a passion for patient-centered care.

Key Responsibilities:

• Conduct telephonic outreach and follow-up with patients

• Develop and maintain personalized care plans in collaboration with patients and providers.

• Monitor patient progress, medication adherence, and symptom management.

• Coordinate care across multiple providers and services.

• Document all patient interactions and care plans in the electronic health record (EHR).

• Educate patients on disease management, lifestyle changes, and preventive care.

• Identify and address barriers to care, including social determinants of health.

Qualifications:

• RN license in Iowa.

• Minimum of 2 years clinical experience, preferably in chronic disease management, primary care, or case management.

• Experience with EHR systems and care coordination tools.

• Strong interpersonal and organizational skills.

• Ability to work independently and as part of a multidisciplinary team.

Self-motivated, dependable, and driven to take initiative.

• Passion for helping others and making a positive impact in patients' lives.
