    Salary: $1080 / Week

     

    The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

    Minimum Requirements:

    + Current RN licensure in state practicing

    + At least one year of Case Management experience preferred

    + Current CPR if applicable

    + TB questionnaire, PPD or chest x-ray if applicable

    + Current Health certificate (per contract or state regulation)

    + Must meet all federal, state and local requirements

    + Must be at least 18 years of age

     

    Hospital Case Manager RN – Mission Hills, CA

     

    Amergis Healthcare Staffing is seeking experienced Registered Nurses for a Hospital Case Manager role at a respected acute care facility in Mission Hills, CA. This position focuses on care coordination and discharge planning for hospitalized patients, ensuring safe transitions and optimal outcomes.

     

    Key Responsibilities:

    + Perform comprehensive assessments and develop individualized discharge plans.

    + Collaborate with interdisciplinary teams to coordinate patient care.

    + Ensure timely and appropriate transitions across levels of care.

    + Advocate for patient needs and facilitate access to resources.

    Schedule & Pay:

    + Shift: Day | 5x8-Hour | 08:00 AM – 4:30 PM

    + Weekend Requirement: Every other weekend

    + Travel RN Weekly Pay: $3,019/week

    + Includes $1,080 taxable income + $1,939 stipends

    + Local RN Pay: $60/hour

    Qualifications:

    + Active RN License – State of California (Required)

    + Minimum 2 years of acute care hospital case management experience

    + Strong background in care coordination and discharge planning
