    Job Description

    The Director of Care Management is responsible for the strategic oversight and operational leadership of care coordination, utilization management, and discharge planning across the healthcare system. This role ensures the delivery of high-quality, patient-centered care that is efficient, cost-effective, and aligned with regulatory standards and organizational goals. The Director collaborates with multidisciplinary teams, including physicians, nurses, social workers, and administrative staff, to develop and implement care management programs that support optimal patient outcomes and transitions of care. They analyze performance metrics, lead quality improvement initiatives, and ensure compliance with federal, state, and accreditation requirements. Additionally, the Director plays a key role in staff development, budget management, and fostering a culture of continuous improvement and accountability within the care management department.

    Over 5 years of hospital care management experience

     

    Over 3 years of leadership experience
