The Behavioral Health Home (BHH) care management program has an employment opportunity open! This is a full-time office (1.0) split position based out of Grand Rapids. Position: Qualified Health Home Specialist (0.5)
Primary Function: The Qualified Home Health Specialist serves as a support to assist clients and team members in reaching the client's goals to achieve optimal health and an improved quality of life.
QHHS: Duties and Responsibilities:
Collaborate with other service providers.
Assist clients in completing paperwork, applications, and forms.
Arrange, obtain, and transport clients as needed.
Assist clients with setting up, preparing for, and attending appointments. Assist with follow-up.
Complete requests, receipts, and tracking for medical records.
Provide resource awareness and access supports to address client's goals and needs.
Work in partnership to assist with other BHH team member caseloads and DHS requirements.
Support BHH team in managing DHS requirements and internal quality systems.
Document according to NCC policy.
Other duties as assigned by supervisor.
Position: Systems Navigator (0.5)
Roles/Responsibilities:
Manage a shared case load of up to 28 clients.
Complete DHS assessments within time restrictions to identify and evaluate goals.
Connect clients to resources to manage medical, social, and mental health conditions.
Assess for and provide culturally competent care to clients.
Provide flexible support to advocate for and assist client in achieving goals to attain optimal health.
Assist clients with setting up, preparing for, and attending (this may include going along with) appointments. Navigate the healthcare system and coordinate care to ensure follow-up.
Document according to NCC policy.
Complete ongoing training assigned by NCC or required by DHS.
Other duties as assigned by supervisor.

Develop and/or provide In-service training to staff as requested.
Maintains an active license status – for licensed professionals
Provide services and conduct self in a manner consistent with the professional Code of Ethics.
Assess or provide therapy, counseling, intervention and/or education to clients in a professional manner.
Provide clinically appropriate treatment based on mental health assessment and treatment plans.
Complete, timely and accurate documentation.
Participating in Clinical Supervision, as needed, by a board approved licensed provider
Participate in planned supervision and case consultation conferences.
Ability to maintain active caseload set forth by your direct supervisor.
Participate in open-access rotation.
Maintain compliance with needs
Attendance and/or completion of mandatory staff meetings and/or training
Ensure compliance with all agency license/certifications (Rule 29, CCBHC, program certifications), HIPPA, and other program(s) compliance and facility policies and procedures
Follow dress code as outlined in employee handbook or directed by supervisor
Follow proper reporting procedures as outlined in policies and procedures manual
### Minimum Education Required
Mental Health Practitioner (MHP) or Community Health Worker (CHW)

• A CHW certificate is received within the Minnesota State Colleges and Universities system.
### Minimum Experience Required
Minimum qualifications: Valid driver’s license, insurance, reliable vehicle. Ability to complete and pass a background check. Knowledge of community resources and healthcare providers is preferred. Applicants should possess excellent communication skills and be able to work effectively both independently and as part of a team.
