We provide long-term solutions to the problems of homelessness, addiction, incarceration, and poverty by helping the people we serve overcome obstacles and develop healthier, more self-sufficient lives.
Case Manager - Permanent Supportive Housing
Full Time - M-F; appx. 8:30am -5:00pm
SUMMARY
This professional will provide support services to homeless individuals with one or more disabilities and their families to secure and maintain stable housing and household income.
JOB DUTIES AND RESPONSIBILITIES
+ Provide all aspects of Housing Case Management for a case load in accordance with VOA policies and HUD and community standards.
+ Responsible for completing all HUD funding requirements including but not limited to housing inspections, rent reasonable certifications, and rental calculations at entry and annually.
+ Collect recertification documentation and determine program eligibility.
+ Calculate income and expenses to determine participant rental portions in accordance with VOA and HUD regulations.
+ Schedule regular meetings with participants including detailed home visits at least once per month or more often as needed.
+ Receive and respond to referrals within 48 hours.
+ Schedule intake, confirm eligibility and complete enrollment within two weeks.
+ Develop an individualized service plan for new participants.
+ Complete and update assessment, acuity scale and individualized service plan on biannual basis.
+ Responsible for ensuring participants locate and maintain stable housing.
+ Responsible for ensuring participants obtain, maintain or increase income.
+ Create and maintain a good working relationship with landlords.
+ Provide on-going and relevant pro-social supports and life skills training.
+ Use evidence-based programming to help participants address challenges to stability.
+ Provide transportation to participants as needed.
+ Establish good working relationships with related agencies to maintain a comprehensive referral network for participants.
+ Maintain accurate records for all participants in accordance with VOA policies and relevant regulatory and funding sources, including but not limited to use of HMIS.
+ Prepare reports as requested.
+ Other duties as assigned.
EXPERIENCE AND EDUCATION QUALIFICATIONS
+ Associates Degree in Human Services, Social Work or related acumen and 3 years relevant experience or an equivalent combination of education, employment and life experience.
+ Bachelors degree and at least one year experience preferred.
+ Clean NYS Drivers License
+ Availability to occasional work evenings and weekends as required
+ Strong organizational skills.
+ Working knowledge of MS Office products
+ Flexibility required to respond to emergency participant needs
