    Job Description

    Directs growth, performance and development of Home Care services through the day to day administration, planning, coordinating, managing and directing of all clinical, operational and financial activities of different home based care departments.

    Job Responsibilities

    + Leads a Home Care Services function by developing leaders and driving programs and initiatives that support and impact the overall Northwell health system goals.

    + Develops and articulates a long-term strategic vision for areas of responsibility.

    + Formulates and administers all policies, plans and programs related to the delivery of home care services.

    + Participates in short and long-term strategic planning to help shape the delivery of services to meet emerging trends.

    + Plans and forecasts staffing requirements to support the delivery of all home care services. Ensures compliance with federal, state and local regulations, tax laws and the Joint Commission standards. Interprets and standardizes all fiscal and regulatory reporting standards to ensure compliance with government and regulatory reporting and third-part agencies.

    + Interviews, selects, and continually evaluates staff performance and provides support for professional training, growth and development of staff Establishes clear goals and program objectives and work plans for staff Plans and establishes controls to monitor the budget and financial impact and results of all managed care services.

    + Monitors and ensures the delivery of high quality, state of the art services compliant with applicable healthcare law, applicable licensing and accreditation requirements and related policies.

    + Develops key performance indicators, benchmarks and controls to assure deliverables are met on all contracts and grants.

    + Monitors and manages utilization, satisfaction and accessibility of all services.

    Job Qualifications

    + Masters degree in Nursing, Business Administration, Health Care Administration, or related graduate degree.

    + Current License to practice as a Registered Professional Nurse in New York State required.

    + Minimum eight (8) years respnsible home care nursing and management experience, required.

    + Recent CHHA experience, as well as leadership experience in Quality Management in this environment, highly preferred.

    + Audit experience with The Joint Commission and CHAP, highly preferred.

    The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

     

    The salary range for this position is $152,000-$266,000/year
