**Job Purpose or Objective(s):** As the Case Manager, you will be a source of social, educational, and spiritual support for individuals and families to develop skills and enhance quality of life. You will report to the Departmental Manager or Director.
**Locatio** n: Durant
**Schedule** : Regular/Full-Time - 8:00am-4:30pm
Wage access is an option for this position.
Primary Tasks:
+ You will contact clients to perform intake assessment and analysis.
+ Provide an overview to the client by describing the extent and nature of the services available to them.
+ Provide educational and social case management services to clients including setting goals and developing case plans
+ Review and monitor participant progress and provide resources, advocacy, and referrals.
+ You will manage case to ensure progress and adjust as needs change.
+ Develop working relationships with individual patient and family.
+ Make referrals to community agencies and conduct follow-up activities.
+ Be a patient liaison and advocate.
+ Perform other responsibilities as assigned.
Job Requirements:
+ Bachelor’s degree in human service, social work, or education field and one [1] year directly related experience, OR five [5] years of directly related experience in lieu of education.
+ Good social skills and ability to build trust and relationships.
+ Good oral and written communication skills
+ Basic knowledge of Microsoft Word and Excel
