What You’ll Do:

+ Conduct comprehensive patient evaluations and reassessments

+ Develop and implement individualized therapy plans in collaboration with physicians and care teams

+ Provide hands-on physical therapy treatments and monitor patient progress

+ Educate patients and caregivers on therapeutic exercises and equipment use

+ Supervise Physical Therapy Assistants and participate in interdisciplinary case conferences

+ Maintain accurate, timely documentation in accordance with care plans and policies

What We’re Looking For:

+ Licensed Physical Therapist in the state of practice

+ Graduate of an accredited Physical Therapy program

+ Minimum 1 year of experience preferred (acute care, outpatient, home health, or community health)

+ Current CPR certification and up-to-date PPD or chest x-ray

+ Strong communication skills and computer proficiency

+ Compassionate, collaborative, and committed to patient-centered care
