    Overview

     

    As a Social Service Staff you will assist the Social Services Director/Coordinator in identifying and providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

     

    How you Will make a difference

     

    + Assist residents and their families with social, emotional, and family issues.

    + Assist in prividing timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.

    + Participate in resident care planning and discharge planning.

    + Serve as a general source of information for residents and their families

     

    What you Need to make a Difference

     

    + At least one (1) year of experience working in social work, preferably in healthcare setting.

    + High level of professionalism and ability to maintain confidentiality.

    + Effective Verbal and Written communication skills.

     

    PART TIME approx 20 hours per week

     

    The part-time Social Worker will be 20 hours.

     

    Tuesday 8 hours

     

    Wednesday 8 hours

     

    Every other Thursday 4 hours

     

    Every other Saturday 4 hours
