Program Coordinator
The Opportunity
The Coordinator provides leadership, supervision, and support to staff within the Health Home Care Management, Non-Medicaid Case Management, and Y-Fi programs. This role ensures that service delivery, documentation, and billing processes comply with all regulatory requirements and that program capacity and reimbursement opportunities.
Responsibilities
+ Provide supervision, guidance, and ongoing support to program staff.
+ Maintain program capacity and optimize reimbursement where applicable.
+ Review and ensure that all Health Home billing complies with regulatory standards and is submitted timely.
+ Enforce and verify the timely completion of progress notes, assessments, and care plans.
+ Complete and submit all county, state, and agency reports within established deadlines
+ Review and verify staff timesheets weekly.
+ Identify and ensure staff training needs are met.
+ Monitor service dollar usage and ensure compliance with related policies.
+ Provide staff with a minimum of monthly supervision and complete timely and meaningful performance evaluations.
+ Conduct weekly or biweekly team meetings and attend monthly program and division meetings.
+ Collaborate with other programs within the Division to maximize referrals and strengthen family support.
+ Provide ongoing outreach to the community and partner organizations.
+ Support agency-wide initiatives and goals as assigned.
+ For Non-Medicaid and Y-Fi programs: maintain and regularly monitor the program waitlist.
Qualifications
+ Master’s degree in a human services field required; LMSW preferred.
+ Minimum of 2 years of supervisory or managerial experience.
+ 3–5 years of direct service experience working with children and youth in the mental health system preferred.
+ Excellent organizational and time management skills, with the ability to multitask and prioritize.
+ Strong interpersonal skills with the ability to build relationships at all levels.
+ Advanced computer skills with MS Office Suite
+ Valid and clean New York State Driver's License.
