About this role:

As an Advanced Practice Provider on our Post-Acute Care Team, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will have some remote-from-home work but primarily will see Post Acute Care - Skilled Nursing patients at the facilities we serve in Salt Lake County.

+ This position includes up **$** **7,500.00 toward eligible** relocation assistance if relocating to Salt Lake City Utah Area

+ You will practice with Intermountain Health at the facilities we serve in the Greater Salt Lake County

+ Mileage Reimbursement available for driving between facilities

+ Your hours will be Monday through Friday, 8am-5pm- to the facilities you are assigned to

+ You will be on call 1:9 rotation for days and weekend

What** **you’ll** **bring:

+ Reliable vehicle and active unobstructed Utah state driver's license required

+ APRN or PA-C degree from an accredited program

+ Training or experience in geriatrics medicine is preferred but not required

+ New Graduates will be considered

+ Board certification or eligibility in the designated discipline

+ Active, unobstructed Utahmedical license, or the ability to obtain one

+ Full prescriptive authority and an active DEA, or the ability to obtain one

+ Ability to successfully complete Intermountain Health’s credentialing process

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$51.61 - $79.62
