    Heritage Manor, a DePaul Senior Living Community is hiring a full-time Case Manager.

     

    The pay range for this rolie is $26.65 - $27.55

    Responsibilities

    The areas of responsibility will include but are not necessarily limited to:

    + Respects and maintains resident rights and confidentiality and demonstrates a caring, positive attitude toward all residents, staff and guests and service providers.

    + Assumes primary responsibility for coordinating and performing services necessary to provide residents’ with an optimal living situation taking care to enhance their autonomy and socialization skills.

    + Works with the Administrator to develop and implement an on-going marketing plan, enhancing awareness and educating service providers and the community-at-large re; the Community and ensuring a sufficient number of applicant referrals and admissions to maintain budgeted census.

    + Conducts interviews and evaluates prospective residents to determine appropriateness; tours families and applicants and oversees the complete admission process for all new admissions, including potential residents of the Assisted Living Program.

    + Works with residents to encourage healthy connections with family and community as well as develop new interests and form new support systems.

    + Maintains accurate and timely records of case management interventions, appointments, recommendations and follow-up in each residents file.

    + Provides on-to-one support for residents related to emotional, behavioral, social and emotional concerns.

    + Works with the Resident Care Director to ensure that the residents obtain / maintain a primary physician of their choice and has access to other services as needed to maintain their optimal health / mental health.

    + Actively networks and maintains links with other health care and human service organizations. Makes appropriate referrals according to resident’s needs.

    + Assists residents in need of alternate living arrangements to make and execute sound transfer and / or discharge plans.

    + Performs other duties as assigned and is available as needed to ensure that all resident’s needs are met.

    ESSENTIAL FUNCTIONS:

    + Ability to communicate with residents and staff in English.

    + Ability to read and write English.

    + Ability to lift up to 30 pounds unassisted.

    + Ability to visually observe residents, to hear emergency systems and verbal needs expressed by residents.

    + Ability to turn, stoop, bend and stretch in order to assist residents.

    + Ability to stand for prolonged periods.

    + Ability to use equipment competently.

    + Ability to respond to emergencies, i.e. resident choking, evacuation of residents and visitors in the event of fire.

    Qualifications

    + Must be at least 18 years of age.

    + Bachelor’s degree with major course work in human servicesplusone (1) year full-time work experience in a related field;oran Associate’s degree with major course work in human servicesplusthree (3) years of full-time work experience in a related field,ora High School diploma or GEDplusfive (5) years of full-time work experience in a related field

    + Well-developed organizational skills.

    + Ability to work effectively with inspectors, monitoring and licensing agencies.

    + Must have the required annual Tuberculosis (TB) testing and annual Health Assessment completed

    + Must have the required references and criminal record check.

    + Must have a valid New York State driver’s license that meets agency’s clean driving record policy.
