As the Licensed Practical Nurse, you will play a pivotal role in individuals' lives, making a significant impact through:
+ Compile monthly/quarterly statistics and submit them to the Health Care Coordinator.
+ Be available for after hour consultations regarding medical concerns.
+ Enter all medical information from all doctor appointments onto the computer as well as into the CONNEX system.
+ Document all medical information in the client's medical chart.
+ Send all medical information in to youth’s respective counties in accordance with our current contracts (ie: email to Albany County.)
+ When clients return from the appointment, note and schedule any lab tests, x-rays, referrals and follow-up on appointments. Make Residential Manager aware of all appointments to ensure proper staffing and transportation.
+ Must keep an inventory of all medical supplies and keep the medical closet organized. Provide proper supervision to ensure the health, welfare and safety of clients.
+ Provide transportation on an as needed basis.
Licensed Practical Nurse Minimum Salary: $ 28.00/hour
Licensed Practical Nurse Maximum Salary: $ 32.00/hour
Licensed Practical Nurse Schedule: Monday - Friday (35 hours per week)

Qualifications:
+ High School Diploma or GED
+ LPN Certification
+ Valid Driver's License
+ 5+ years in a residential setting and working with a diverse population
