    Job Summary

    Novant Health Lakeside Family Physicians Prosperity Church The Licensed Practical Nurse serves as a member of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with other team members to provide care that promotes optimal health.

     

    The Licensed Practical Nurse utilizes best scientific evidence and compassion to assist the care of the patient during their health journey. The Licensed Practical Nurse accepts accountability for provision of care, as part of the RN led team, in accordance with the current policies and procedures.     Responsibilities

    + Assessment: Participates in the collection of data as directed by the registered nurse. Participates in the determination of nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen. Sources of data collection include: subjective reporting, observations of appearance and/or behavior, measurements of physical structure and physiologic function, information regarding available resources. Able to determine a patient's need for immediate nursing intervention based upon data gathered regarding the patient's health status, ability to care for self, and current treatment regimen.

    + Planning: Participates in dialogue with the patient/chosen support system to identify what is important to them and incorporate his/her preferences and norms into the individualized care plan. Participates as part of the interdisciplinary care team in planning individualized care. Identifies relevant resources based on necessity and availability. Identifies nursing interventions and goals for the patient and reviews goals with the registered nurse. Collaborates with the registered nurse to consider ongoing modification of care plan based on patient’s needs, capabilities and condition.

    + Evaluation: Participates in evaluation of nursing care and desired outcomes by collecting evaluative data from relevant sources; recognizing the effectiveness of nursing interventions; and proposing modifications to plan of care for review by the registered nurse.

    Qualifications

    + Education: High School Diploma or GED, required. Graduate of accredited Practical Nursing Program

    + Experience: One year of relevant experience, preferred.

    + Licensure/Certification: Appropriate state LPN license, required.

    + Additional Skills (required): Computer skills, Phone call etiquette, Organizational Skills.
