    Job Summary and Responsibilities

     

    At CHI Health, we recognize that Social Determinants of Health (SDOH) can significantly influence patient outcomes. As a Social Worker – Outpatient Care Coordinator, you’ll play a key role in addressing the social and emotional needs of patients and their families, helping them access resources, overcome barriers, and achieve better overall health.

     

    Working within our outpatient CHI Health Clinics, you will be part of a multidisciplinary care team and collaborate closely with physicians, staff, and CHI Health Partners to improve care quality, patient experience, and outcomes.

    What You’ll Do

    + Identify patients who would benefit from care coordination and social work support.

    + Complete psychosocial assessments and serve as an advocate for patients and families.

    + Connect patients to community resources and services to meet economic, behavioral, or social needs.

    + Provide health education, coaching, and support to promote independence and problem-solving skills.

    + Collaborate with physicians, nurses, and care teams to eliminate gaps in care and ensure smooth care transitions.

    + Support population health goals by identifying opportunities for improvement at the patient, clinic, and system levels.

    + Act as a liaison between patients, families, and healthcare providers to enhance communication and continuity of care.

    + Carry out health promotion activities to empower patients and improve long-term health outcomes.

    **Education:** Bachelor's or Master's Degree from a school of social work accredited by the Council of Social Work Education

    **License/Certification:** Social Work

    Pay Range

    $22.51 - $31.79 /hour
