The Clinical Pharmacy Practitioner functions as a therapeutic expert in drug use and drug utilization. The incumbent monitors drug use makes recommendations on the correct utilization of drugs, exercises duties as outlined in their scope of practice and educates medical center staff and providers on drug utilization. Responsibilities VA Careers - Pharmacy: https://youtube.com/embed/Fn\_ickNBEws Participates in Hematology/Oncology Inpatient rounds Responsible for daily performance and oversight of provider-consulted, clinical pharmacy pharmacokinetic monitoring program for hematology/oncology patients. This includes the rational design of a therapeutic dosing regimens based on patient-specific information that includes selection of an appropriate drug dose and establishment of a therapeutic endpoint. Duties include progress note entry, mediation order entry, lab/monitoring follow-up and dose titration as appropriate for all Veterans in the inpatient wards receiving chemotherapeutic agents. Responsible for daily performance of clinical pharmacy chemotherapy monitoring program. This includes ordering of appropriate laboratory monitoring, rational dosing titration and progress note entry for all patients receiving chemotherapeutic infusions based on patient specific information and establishment of a therapeutic endpoint. Assesses patients on parenteral and enteral nutrition. In conjunction with nutrition, parental formulations are reviewed for appropriateness based on laboratory parameters. Documents parenteral formulations daily in the electronic medical record. Assesses patients on enteral nutrition for medication-related problems and recommendations for changes are communicated and documented in the electronic medical record Reviews medication profiles and summarizes significant findings which are documented in the medical record and reported to the primary provider. Identifies any drug-induced problem that may be affecting the patient or contributing to the need for hospitalization, including adverse effects, allergic reactions, toxicities, improper dosage regimen design, noncompliance, etc. The incumbent is expected to perform duties with strict adherence to all Pharmacy Service, Medical Center, VHA, and other regulatory agencies policies, procedures and regulations. Has good understanding of DEA laws concerning the dispensing of controlled substances. Plans and coordinates formal and informal instruction/orientation to all staff within area of assignment Provides patient education about medications and related topics when appropriate. In cooperation with clinical section staff and other health professionals collects and analyzes data related to ongoing clinical programs and presents it either at national meetings or publishes it in professionally representative publications Serves as preceptor of residents' research projects as assigned. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the 2 graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet Based Test (TOEFL iBT) Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Candidates must demonstrate an ability to function independently and effectively in a collaborative acute care environment and meet qualifications as a PGY2 preceptor. Competitive candidates will have completed a PGY2 Hematology/Oncology -or- have completed a PGY1 residency with significant hematology/oncology experience. Candidates without residency training but have extensive clinical experience in hematology/oncology may be considered. BCOP certification is highly desirable.
