Summary James A Haley Veterans Affairs is looking for Senior Social Worker -Emergency Department, who will report to Social Work Service, and is assigned to the Continuity of Care Program. The JAHVAH is a Level I tertiary medical center with general medicine, surgical, psychiatric, rehabilitation, spinal cord injury, nursing home care beds and one of five Polytrauma treatment facilities in the VA enterprise. Responsibilities Total Rewards of a Allied Health Professional The Senior Social Worker is a member of the Social Work Service Continuity of Care Section. The Senior Social Workers provide advanced clinical psychosocial and case management services to Veterans and their families to address biological, social, and environmental needs. They offer social work access for Veterans receiving acute and emergent hospital care at the Tampa VA hospital during hours of operation. The services they provide include comprehensive assessments, care coordination, supportive counseling, disposition and discharge planning, and other relevant services for Veterans and their families, all in line with accepted standards of practice. Additional Duties include but not limited to: Conduct psychosocial evaluations, mental status examinations, and functional ability assessments for complex, high-acuity patients. Engage in treatment planning with multidisciplinary team members in hospitals and the community. Provides community care options based on available resources and each Veteran's benefits. Demonstrate advanced knowledge in risk and crisis assessment. Offer consultation and guidance to colleagues regarding social determinants of health that do not require hospital admission. Educate multidisciplinary team members and medical residents on social work practice standards, as well as VA policies and procedures. Provide education on internal and community resources, the care transition process, Goals of Care conversations, advance directives, and VA benefits that affect healthcare options, including social determinants of health status. Exhibit advanced skills in a range of specialized interventions and treatment modalities for patients facing acute illness episodes, chronic illness, and end-of-life concerns. Engage in accepted standards of social work practice, including individual, family, and group counseling, to help Veterans and their families cope with grief, loss, terminal illness, and death. Participate in clinical peer reviews and other credentialing tasks in accordance with VA and regulatory policies. Take part in peer reviews, case conferences, committee assignments, and discipline-specific meetings as needed for professional growth and development. Conduct comprehensive analyses of social determinants of health and their influence on quality of life. Demonstrate proficiency in developing and implementing methods for measuring the effectiveness of interventions. Additionally, exhibit the ability to make informed modifications based on empirical findings and outcome evaluations. Maintain section databases, dashboards, and SharePoint in accordance with VA mandates and directives. Document social work interventions in Veterans' medical records following hospital policies and regulatory guidelines. Contribute to the social work service on-call rotation, which may include adjusting work hours to meet patient care needs during holidays, evenings, and weekends. May provide supervision to a Social Work intern or participate in the MSW training program. Additionally, may offer orientation to less experienced Social Workers. Other duties as assigned. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Senior Social Worker candidates must have the following: Experience/Education: The candidate must have at least two (2) years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one (1) year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Acute care social work or emergency room experience in VHA. VHA Computer Patient Records System knowledge. Experience in crisis intervention, care coordination, care management, disposition, and discharge placement is required. Be able to work in a fast-paced environment due to high acuity, large patient volume, and rapid patient turnover. Essential to have the ability to effectively triage, complete a comprehensive needs assessment, and understand social determinants of health that can be managed on an outpatient basis. The role aims to prevent social admissions whenever possible. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
