Summary This position is in the Physical Medicine and Rehabilitation Service of VANJHCS. The Physical Therapist will spend majority of work time administering administrative and treatment programs related to the care of the Geriatric patient. Working on a multidisciplinary team, representing the Physical Therapy Section by communicating directly with physicians, nursing personnel social worker, dietitian, psychologist, pharmacist and other disciplines involved with the patient's care. Responsibilities Duties include but are not limited to: Will be required to work at both the East Orange and Lyons, NJ Campuses. Clinical Recognition of age, development and cultural needs of patients. Performs physical therapy evaluation and treatment with hospitalized patients, outpatients and employees, male and female, ages ranging from adult to geriatric, the majority being geriatric, having a variety of disabilities. These cases may be complex in nature with chronic and acute disease entities including but not limited to central nervous system disorders, joint diseases, circulatory and pulmonary diseases, diabetes mellitus, altered thought processes and multi system disease combinations. Has ability to determine the most appropriate plan of care relative to the patient's diagnosis and age. Skills were obtained through formal education and are maintained by continuing education. Serves as role model for students and others on the physical therapy staff. Employs unusual motivational techniques to enlist patient participation and adapts equipment and procedures to carry out a full range of complex treatment activities. Evaluates patient's condition upon appropriate referral to physical therapy to determine treatment plan. Refers the patients to the Physical Medicine and Rehabilitation Service physician when determined to be necessary. Performs and interprets results of tests and measurements for use in preparing for modification and/or evaluation of patient treatment, and adjusts the treatment based on changes in patient's condition. Evaluates patient's needs with respect to prescribed braces, orthotics and prostheses. Selects appropriate durable medical equipment, determines which suits the patient best, and recommends necessary modifications to provide for proper fit. Demonstrates advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of Physical Therapy. This knowledge has been acquired through at least 2 years of clinical experience in this specialty area, or through board certification, clinical residency, Fellowship, or an advanced degree in a related field. Demonstrates ability to serve as a consultant for health care providers regarding the delivery of care within the geriatric physical therapy population. This would include the ability to teach and mentor medical center staff in this specialized area of Physical therapy. Has the ability to guide and evaluate the work of a multi-disciplinary team in this specialized area of physical therapy. - Demonstrates advanced skill to perform and interpret specialized evaluative procedures and tests in the evaluation and treatment outcomes in the geriatric specialty area. Education Identifies continuing education needs based on performance of physical therapy staff regarding geriatric care on an as needed basis. Provides continuing education in the form of in services and clinical training programs to physical therapy staff on subject matter of geriatrics, on an as needed basis. Serves as subject matter expert in the area of geriatrics and as a consultant to physical therapy and other medical center staff in evaluating and treating geriatric patients. Administrative Responsibilities: Receives patient referrals from an appropriate source and schedules patients for evaluation and treatment in a timely fashion. Reviews charts to assess the appropriateness of the referrals and possible contraindications for treatment. Schedules patients for treatment, considering priority and frequency of care and coordinating schedules with other members of the treatment team. Flexibility of the schedule must be maintained to allow for unscheduled patients and for family conferences. Documents performance and timely treatment and achievement of treatment goals assuring a high level of productivity and cost-effective treatments are provided. Establishes and maintains an appropriate QA monitor relating to geriatric patients as/when needed. Attends interdisciplinary treatment team meeting when needed to provide input with regards to patient progress and discharge potential when indicated. Keeps track of therapy minutes and enters data into the Minimum Data Set. Assures all documentation is done timely on all patient visits. Performs other duties as assigned. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and experience. The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy NOTE: Prior to 1995 the terminal degree for PT's was either a bachelors or master's degree. In 1995 CAPTE accredited all baccalaureate, master's, and Doctor of Physical Therapy terminal degree programs. Effective 2002, CAPTE changed its scope of accrediting activities of PT education programs to include only those programs that culminate in postbaccalaureate degrees(eliminating the baccalaureate degree). Effective 2015, professional physical therapy education programs have only offered the Doctor of Physical Therapy (DPT) degree to all new students. NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org. Loss of Credential. A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists. Grade Determinations: GS-13 Physical Therapist (Clinical Specialist) (a) Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level. (b) Demonstrated Knowledge, Skills, and Abilities. Candidates must demonstrate all of the KSAs below: 1. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy. 2. Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy. 3. Ability to teach and mentor medical center staff in a specialized content area of physical 10 therapy. 4. Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy. 5. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area. (c) Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety. Clinical Specialists spend at least 25% or greater of their time working with specialized population(s) within their department. Physical therapists at this level perform assignments in specialty areas such as geriatrics, neurology, cardiopulmonary, orthopedics, spinal cord injury, amputee, wound care, chronic pain, electromyography, women's health or pelvic health, polytrauma, brain injury, lymphedema, vestibular, assistive technology, seating and mobility. The individual is assigned responsibility for serving as the subject matter expert in the content specialty area and as a consultant to physical therapy and other medical center staff in evaluating and treating patients in the specialty area. Clinical Specialists have advanced knowledge and demonstrate expert clinical practice skills in providing assessment and treatment interventions across the continuum of care. He/she utilizes advanced knowledge and expertise to modify treatment plans and to identify therapeutic activity interventions based on the changing needs, goals and performance of patients. Clinical Specialists have advanced training in specialized technology related to their area of practice. He/she establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis. He/she exercises expert professional judgment to establish appropriate discharge planning recommendations to ensure safe discharge from inpatient or outpatient rehabilitation programs. If the medical center has a PT residency program, the clinical specialist may be on faculty for the program. Preferred Experience: Geriatric related certification and participation in interdisciplinary team meetings. Reference: VA Handbook 5005/99 Part II Appendix G13.
