    JOB SUMMARY

    Represents a family or caregiver of a client with mental health or substance use disorders in all aspects of treatment within all systems of care a family is involved with.

     

    Utilizing skills transfer methods, you will, promote engagement with clients empowering them to problem-solve and identify their individual goals, solutions, and routes toward successful recovery.

     

    The ideal candidate for this role should be flexible and able to manage multiple priorities.

    MINIMUM QUALIFICATIONS

    One (1) year of closely related experience to this position and experience supporting individuals with special needs and substance abuse.

     

    Must possess a valid Driver's License issued by the State of Utah and own transportation to and from various job locations.

     

    Due to the nature of this position, the successful applicant must pass a required background investigation. Preference given to candidates who are bilingual or multilingual and/or working with the LGBTQ community. Also, preference given to candidates who have experience in Medicaid billing or documentation.

     

    Experience navigating child serving agencies is preferred.

    ESSENTIAL FUNCTIONS

    + Conducts intake processes.

    + Assesses families' needs based on the intake information.

    + Advocates for families with community-based resources.

    + Provides resource referral services.

    + Collects data and provides reports to the funding agency to meet the contractual requirements.

    + Monitors the billable Medicaid services for the program.

    + Attends required trainings and meetings.

    + Maintains confidentiality and adheres to Health Insurance Portability and Accountability Act (HIPAA) requirements.

    + Ensures required services documentation is accurately completed in a timely manner.
