    CCSI is contracted by Onondaga County to provide program management services, including specialized staffing. The Community Case Monitor I will be in Onondaga County and is responsible for all aspects of monitoring Assisted Outpatient Treatment (AOT) Enhanced Agreements, AOT Court Orders, and Forensic Transition Cases. The position will provide support and oversight for high-needs individuals with serious mental illness who have a history of medication non-compliance and who live in Onondaga County. The case monitor will coordinate and facilitate multi-disciplinary teams that include: the client, behavioral health treatment providers, case management, and housing supports, as outlined in the treatment plan. Supervision will be provided by the Forensic Case Monitor Supervisor, in close collaboration with the AOT Coordinator and Director of Forensics.

    In this role, you will be:

    + Collaborates and assesses needs of individuals referred to and assigned by the AOT Coordinator and the Manager of Forensics.

    + Coordinates the development of system-wide treatment and support plans to include treatment services, case management, housing, care management, and other identified supports.

    + Monitor client’s participation in and compliance with an established treatment plan.

    + Schedule, plan, and facilitate team meetings.

    + Communicates effectively with the client, providers, families, and criminal justice personnel to assist with developing and sustaining the plan.

    + Creates a sustainable treatment team by assigning roles and tasks to the treatment team to prepare for the transition to a lower level of care.

    + Monitors treatment implementation and coordinates action when treatment, service, and support plan needs adjustment.

    + Recommend adjustments to treatment plans, and advocate for system movement.

    + Track individuals across multiple systems of care over time to assure continuity of linkages and supports.

    + Works closely with treatment providers, ACT, case management, housing providers, parole, probation, Peer Bridger, and unique services and supports.

    + Culturally responsive to the population being served in the development, design, monitoring, evaluation, and/or implementation of programs and services.

    + Organizes interviews or meetings with applicants and providers involved in the plan as needed.

    + Cultivates partnerships with community providers to best support clients.

    + Supports community education requests.

    + Request and review clinical records.

    + Initiate, assist in, and complete investigations for appropriateness of AOT

    + Attend staff and other community meetings.

    + Maintains and organizes electronic and paper records, and updates tracking sheets. Records progress notes in a comprehensive, timely manner.

    + Identify creative solutions to complex problems utilizing available community resources

    + Supports clients in achieving their program goals.

    + Develop, track, and report program goals and objectives.

    + Travels as necessary to fulfill responsibilities.

    + File maintenance for the program, including filing and archiving of records.

    + Other duties as assigned.

    Requirements

    What You Bring:

    + Bachelor’s degree in Human Services field plus two years of mental health experience.

    + Must have a valid NYS driver’s license and reliable transportation.

    + Satisfactory completion of all required background screenings: criminal, and driving.
