The Program Coordinator serves as the primary case management and program operations lead for the Medical Respite program. This role is responsible for coordinating client care and ensuring that individuals experiencing homelessness receive safe, supportive, and medically appropriate services.
POSITION RESPONSIBILTIES
+ Conduct comprehensive client intake assessments, eligibility screening, and triage including at site and in the community
+ Develop, implement, and monitor individualized care plans in collaboration with medical, behavioral health, and social service providers.
+ Provide ongoing case management to address medical, behavioral health, social, and housing needs.
+ Facilitate discharge planning and ensure warm hand-offs to community resources, including permanent housing, primary care, and supportive services.
+ Track client progress, document services provided and maintain accurate records in compliance with regulatory and funder requirements.
+ Support staff in managing complex cases, crisis situations, and client advocacy.
+ Oversee daily operations, including client admissions, discharges, and service delivery schedules.
+ Coordinate with program leadership and clinical staff to maintain optimal program flow and occupancy.
+ Manage resources, supplies, and operational logistics to support program delivery.
+ Ensure adherence to safety protocols, infection control procedures, and organizational policies.
+ Collect and analyze program data related to client outcomes, service utilization, and operational efficiency.
+ Monitor key performance indicators (e.g., length of stay, readmission rates, client satisfaction).
+ Collaborate with hospitals, shelters, healthcare providers, and social service agencies to coordinate referrals and client care.
+ Serve as a point of contact for community partners regarding case management and program operations.
+ All other duties as assigned
POSITION REQUIREMENTS
+ Master's degree in social work or a related field plus 1 year of full-time relevant experience OR
+ Associate or Bachelor of Science in Nursing (ADN/BSN) with RN license plus 1 year of full-time relevant experience OR
+ Bachelor's degree in any field with significant coursework or experience in human, social, or health services plus 3 years full-time relevant experience, including 1 year in program operations or management. OR
+ Associate degree plus 4 years of full-time relevant experience, including 1 year in program operations or management. OR
+ High school diploma or equivalent plus 5 years of full-time relevant experience, including 1 year in program operations or management.
+ Valid Current NYS Driver's License in good standing
+ Knowledge of social determinants of health, community resources, and healthcare systems
+ Experience working with Microsoft Word, familiarity with email applications, comfortable using Electronic Health Records
+ Prior experience in food pantry operations, food service, or community services preferred
+ Physical Requirements: Sits, stands, and moves intermittently during work hours, lifts/pushes/pulls 10-20 pounds daily, lifts 50 pounds occasionally, drive agency vehicle
POSITION RELATIONSHIPS
Reports to: Administrator
SCHEDULE: Monday- Friday 8:30am-4:30pm with on-call responsibilities
PAY RATE: $22.00 per hour
