    The role you'll contribute:

    Participates in patient assessment, counseling, support, and resource linkage. Provides counseling to patients and families as well as organizing and leading support groups. Works closely with physicians and other medical staff to aid patients in understanding treatment options, facilitating decision making, as well as providing a variety of other case work services in acute care areas, outpatient, and in the community.

    + Provides referrals for patients within the agency and community.

    + Works directly with patients, family members, physicians, and community providers to ensure collaborative and continuous care.

    + Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns.

    + Participates in multidisciplinary treatment team meetings, providing clinical input.

    The expertise and experiences you'll need to succeed:

    Required qualifications:

    + Masters Degree in Social Work, Counseling or related

    + One year of related clinical experience

    + Current State License as a Clinical Social Worker (CSW), Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC)

    + Current Basic Life Support from the American Heart Association
