The UW Medicine Center – Northwest Ambulatory Pharmacy Department** has an outstanding opportunity for a **Clinical Pharmacist – Anticoagulation Clinic.
WORK SCHEDULE
• Part-Time / 20 hours per week
• Day Shift
POSITION HIGHLIGHTS
• Manage anticoagulant therapy for ambulatory patients, working with a team of specialists in antithrombotic therapy and multidisciplinary providers from numerous specialties
DEPARTMENT DESCRIPTION
UW Medicine's **Pharmacy Department** provides high-quality and comprehensive pharmacy services to our patients and providers. Our team of specially trained pharmacists and technicians work closely with doctors and nurses to ensure our patients receive the best medication therapy for their condition.
PRIMARY JOB RESPONSIBILITIES
Drug therapy monitoring, including, but not limited to:
• Implement, monitor, and adjust antithrombotic pharmacotherapy for achievement of desired therapeutic outcome.
• Ensure appropriate drug therapy, including indication, dose, side effects, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events; intervene as needed.
• Provide patient education or initiation of anticoagulant therapy, including bridge therapy and management of drug interactions and adverse outcomes.
• Provide anticoagulation dosing recommendations, drug information, and education to medical teams, nursing, and other healthcare students and staff.
Strong candidates will have completed an ASHP-accredited residency program and/or fellowship (or equivalent experience) and have previous anticoagulation experience.
REQUIRED POSITION QUALIFICATIONS
One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
LEGAL REQUIREMENT
State of Washington Pharmacist License.
