EDDY SKILLED NURSING FACILITIES (located at St. Peter's Hospital, Albany)
The Eddy's highly regarded Skilled Nursing Facilities have a unique opportunity for a RN to receive, review and process referrals of patients at St. Peter's Hospital referred for subacute rehab services. Primary responsibility is to provide on-site support and assistance to hospital case management and hospitalist team to ensure timely identification of inpatients appropriate for referral to subacute rehab and long term care. Upon referral, RN is responsible to screen to determine medical appropriateness and coordinate with the team to verify insurance and obtain payer prior authorization with goals of ensuring a timely discharge and smooth transfer of care to our facility.
Principal Responsibilities & Duties
Candidate Identification & Screening
+ Identify potential rehabilitation candidates early.
+ Conduct on-site and/or telephonic assessments and record reviews.
+ Collaborate with physicians, nurses, therapists, social workers, and case managers.
+ Interview family members or significant others when clients are unable to participate.
+ Assess pre-morbid functional status, personal interests, cultural considerations, and discharge plans.
Clinical Assessment & Admission Coordination
+ Determine medical stability and readiness for rehabilitation in collaboration with PM&R physicians.
+ Ensure recommended services are appropriate, reasonable, and necessary.
+ Facilitate benefit eligibility and precertification with payers.
+ Coordinate smooth transitions from referring facilities to rehabilitation settings.
+ Complete all required preadmission screening documentation.
Education & Outreach
+ Educate clients, families, referral sources, payers, and interdisciplinary teams on program services and benefits.
+ Serve as a rehabilitation consultant regarding therapy progress, discharge planning, equipment needs, and care transitions.
+ Develop relationships with referral sources and conduct outreach activities (e.g., tours, exhibits).
+ Provide information to facilitate referrals and admissions.
Case Management & Communication
+ Act as liaison between internal and external case managers.
+ Develop cost-effective care plans tailored to each client.
+ Demonstrate effective communication with all stakeholders.
+ Maintain high ethical standards and customer service excellence.
Knowledge & Compliance
+ Maintain thorough knowledge of rehabilitation settings, post-acute services, acuity trends, and outcomes.
+ Demonstrate understanding of regulations and payer requirements.
+ Utilize multiple Electronic Medical Record (EMR) platforms.
Other Duties
+ Perform other related duties as assigned.

Experience and Requirements
+ Active Licensure as a registered nurse, preferably with a **bachelor’s degree** in nursing from an accredited school and Certified Registered Rehab Nurse preferred (CRRN).
+ A minimum of 2 years of clinical experience; experience in the rehabilitation of chronically or catastrophically ill or injured individuals
+ Demonstrated expertise in analysis, decision making, time management, oral and written communication, and computer use
+ Excellent assessment skills and knowledge of insurance and managed care principles and practices
+ Ability to function independently
+ The position requires a moderate amount of physical work - individual must be able to maneuver throughout halls, stairways, and patient rooms. Interpretation of environmental input requires visual and auditory skills
+ Ability to make trips to various departments/hospitals will be required
+ Clean valid driver's license required
+ Ability to utilize multiple Electronic Medical Record Platforms
Pay Range: $35.00 - 47.75 per hour
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
