You’ll provide support, guidance, and resources, helping patients get the care they need while connecting them to the community services that make life easier.

What You’ll Do:

+ Offer crisis support, counseling, and resource referrals for patients and families

+ Help plan safe discharges and transitions of care

+ Coordinate with healthcare teams to keep patients supported and informed

+ Connect patients to community resources for housing, food, transportation, and more

Job Requirements

**Education:** Bachelor's or Master's Degree (or willing to enter a Master's Degree program) from a school of social work accredited by the Council of Social Work Education REQUIRED

**License/Certification:** Social Work Certification REQUIRED

At CHI Health CUMC-Bergan Mercy, our focus is patient-centered care. Our level I trauma center and academic medical center campus is designed to heal the body, mind, and spirit of every person in a more comfortable, less stressful hospital environment. Our full range of medical services includes trauma services, heart and vascular care, emergency services, surgery, maternity, cancer care, and diagnostic imaging.

Pay Range

$24.27 - $36.10 /hour
