    Overview

     

    Westchester Jewish Community Services (WJCS), a nonprofit founded in 1943, has grown to be the largest provider of licensed outpatient community mental health services and one of the largest human service agencies in Westchester County. We provide state-of-the-art programs and services and compassionate care to more than 20,000 people of all ages and backgrounds each year.

     

    WJCS is currently seeking a dedicated High Fidelity Wraparound (HFW) Care Coordinator to join our team. This is a full-time opportunity to make a meaningful difference in the lives of youth/adolescents/young adults who are impacted by serious mental health challenges that are affecting their ability to function successfully at home, school, and in the community. HFW is an evidenced-based care management model proven to strengthen families, foster collaboration, and help youth remain safely at home. Training will be provided to become HFW Certified, if not already in possession of such certification.

     

    The salary range for this full-time position is $55,000 - $60,000.

    Responsibilities

    As a valued member of our team, your responsibilities will include, but are not limited to:

    + Effective collaboration to develop individualized care plans.

    + Serving as the point person for each youth’s care, coordinating services and supports.

    + Partnering with a range of providers and family members to ensure success and positive mental wellness.

    + Providing crisis intervention and prevention services as necessary; develop and implement safety and crisis plans.

    + Participating in OMH booster and/or re-certification training requirements.

    + Maintaining fidelity to the HFW process and procedures set forth by OMH.

    + Maintaining accurate and timely documentation.

    Requirements:

    We are seeking candidates who possess the following qualifications:

    + Bachelor’s degree in Social Work or related field with a t least 2 years of relevant experience, or a Master’s Degree with at least one year of experience.

    + Strong ability to engage, support, and empower youth/adolescents/young adults who are impacted by mental health challenges and/or medical needs.

    + Excellent communication and collaboration skills.

    + Valid driver’s license and vehicle are required.

    + Bilingual English/Spanish preferred.

    + NYS High Fidelity Wraparound Certification, or willingness to participate in training to obtain certification.
