    The role you'll contribute:

    Responsible for providing assessment, planning, and instruction to patients with diabetes and their families. This position delivers education for inpatients and outpatients through private sessions and group classes, as well as staff training on diabetes care and treatment. Additionally, this role ensures the delivery of accurate, up-to-date, and effective patient education in collaboration with the patient's primary care physician. In addition, this position oversees the development, planning, and execution of chronic disease management and wellness programming for employees and the community. Adheres to all applicable local, state, and federal regulations, and standards of accrediting bodies.

    + Performs initial assessments to evaluate patient needs related to diabetes management.

    + Develops personalized educational plans for diabetes management and wellness.

    + Provides diabetes education for inpatients and outpatients through private sessions and group classes.

    + Monitors patient progress and provides ongoing support and education.

    + Collaborates with healthcare professionals to ensure comprehensive diabetes care.

    Qualifications

    The expertise and experiences you'll need to succeed:

    Associate's of Nursing Required or

     

    Bachelor's in Dietetics Required

     

    Bachelor's of Nursing Preferred

    Master's in Dietetics Preferred

    1 Work Experience Required

     

    Basic Life Support - CPR Cert (BLS) Required

     

    Registered Nurse (RN) Required or

     

    Commission on Dietetic Registration (CDR) Required

     

    Licensed Dietitian (LD) Preferred

     

    Certified Diabetes Educator (CDE) Preferred
