Forsyth Medical Center, Winston Salem
Full-time, 40 hours/week - Rotating 1st and 2nd shift, every 3rd weekend
*20K SIGN ON BONUS ELIGIBLE*
The Cardiology Care Team pharmacist is responsible for the provision of comprehensive clinical pharmacy services for patients in cardiac intensive care or step-down units including order verification, rounds, heart failure consults, discharge counseling, and pharmacokinetic and therapeutic consultation.
The Pharmacist is responsible for performance and/or supervision of various aspects of the medication use system including but not limited to: therapeutic recommendations, advocate for patient safety, preparation, distribution, monitoring, oversight of administration, and billing. This practitioner will be primarily based on the Cardiology Care Team and may cross-cover other areas as needed. The pharmacist’s focus is on the quality of care through comprehensive medication management.
+ Cardiology experience is preferred
+ Must have residency training PGY1 or comparable acute care experience

Qualifications
+ Education: 4 Year/Bachelor's Degree in Pharmacy, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. PhD, preferred. Doctorate of Pharmacy, preferred.
+ Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
+ Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification, required.
+ Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
