Job Summary and Qualifications
Fulfills pharmacist’s job responsibilities as described in the pharmacist job description.*
Provides a minimum of two (2) in services per year on rational medication therapy and medication procedures effectively to health care professionals and patients.*
Works closely with multidisciplinary team for a particular service, including regular rounds on patients and documentation of rounds in the pharmacy clinical intervention module.*
Serves as the primary facilitator for medication use improvement needs in primary service line.*
Actively participates in committee meetings for primary service line.*
Maintains knowledge with the particular service standard of practice via medical literature and conferences.*
Serves as primary preceptor for pharmacy students and residents in a particular service and fulfills all documentation responsibilities.
Actively participates in the provision of training and education to pharmacists in particular service including pharmaceutical care and disease state management.*
Educates healthcare professionals via written publications/newsletters topics of interest in particular service on an annual basis.
Actively participates as a member of local, state, and national pharmacy societies.
Creates national visibility by presenting clinical information/programs such as poster presentations or managed case studies or publishing in nationally recognized journals.
Serve as subject matter expert (routinely) for LGMC. Serve as subject matter expert (occasionally, usually by phone or e-mail, but occasionally on site) for other hospitals in the market.*
Guides other pharmacists in the promotion of excellent patient care relevant to the specialist’s practice.*
Participates in Division clinical efforts related to specialty.
Implements evidence-based medicine and best practices.*
Supports clinical research and protocol development.*
Utilizes and facilitates research that supports advances in pharmaceutical care.
What qualifications you will need:
EXPERIENCE
Required: At least one year of hospital experience
Preferred: Prior experience with clinical emphasis
EDUCATION:
Required: Bachelor of Science in Pharmacy from an accredited school of pharmacy.
Current license to practice pharmacy in the Commonwealth of Virginia.
Preferred: Doctorate of Pharmacy from an accredited school of pharmacy.
Relevant board certification by the Board of Pharmaceutical Specialties; If the employee is not BPS certified upon hiring, certification must be completed within 3 years.
Hours: Full time, day shift
