About the Job:

Saint Joseph Health System is seeking a compassionate and detail-oriented **RN**

**Case Manager** to coordinate patient care, utilization management, and discharge planning. This role ensures quality outcomes, efficient care delivery, and patient satisfaction through collaboration with physicians, departments, and families.

Key Responsibilities:

+ Manage patient caseloads and coordinate care plans

+ Lead discharge planning and transition processes

+ Facilitate team meetings and resolve care discrepancies

+ Monitor length of stay and level of care using clinical criteria

+ Collaborate across departments to ensure continuity of care

+ Provide education and support to patients and families

+ Maintain compliance with healthcare regulations and documentation standards

Requirements:

+ RN license in the State of Indiana

+ Bachelor’s degree in Nursing (BSN preferred)

+ 3–5 years of clinical nursing experience; critical care experience preferred

+ Case management certification preferred

+ Strong communication, critical thinking, and computer skills
