JOB SUMMARY
Assist with the accuracy and completeness of medication history collection across the continuum of care beginning in the Emergency Department or the inpatient units and under the direct supervision of the Emergency Department.
JOB DUTIES
1. Actively demonstrates the organization’s mission and core values, and conducts oneself at all times in a manner consistent with these values.
2. Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information.
3. Gather information about the patient's medications from family or directly from the patient in a manner that is professional and non-invasive.
4. Collects and compiles a complete list of patient's home medications including over the counter drugs and herbal supplements including drug name, dosage, frequency, compliance and last dose taken.
5. Able to interview to obtain any known drug allergies and reactions.
6. While interacting with patients, patient’s families and guests, as well as other hospital associates and staff, consistently demonstrates SJHS’s core values.
7. Consistently provides superior service by responding to Physician, staff, and/or patient/family requests.
8. Conducts regular rounds of the Emergency Department in order to obtain home medication lists from inpatients and outpatients.
9. Contacts pharmacy, physician offices, etc. to obtain the most accurate medication list as possible.
10. Reconcile final home medication list with patient.
11. Inputs all medication reconciliation information into the computer.
12. Print completed medication reconciliation form to be placed on chart.
13. Participates in staff meetings for the purpose of disseminating information and monitoring associate needs, concerns, and suggestions.
14. Greets patients and families in a courteous, friendly manner making them feel welcome and as comfortable as possible.
15. Demonstrates knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.
16. Performs other duties consistent with purpose of job as directed.
JOB SPECIFICATIONS AND CORE COMPETENCIES
Education: High School Diploma required.
Licensure: Licensed by the Indiana State Board of Pharmacy as a pharmacy technician.
Experience: Four (4) years experience as a pharmacy technician required. Ambulatory or retail experience preferred. Excellent written and verbal communication skills with people from diverse backgrounds and at all levels within the medical center and in the community. Ability to create and foster a cooperative teamwork environment. Basic computer skills, electronic data entry. Knowledge of commonly used prescription drugs including brand/generic names, available dosages, frequencies and indications.
