    Nurse Visit RN/LPN

     

    Job Status : Full-time, Monday - Friday, 8am – 5pm with rotating Saturdays

     

    Sign-on Bonus: $3000.00 (Internal candidates that are currently employed here at NWHS are not eligible for sign-on bonus.)

     

    Referral Bonus : $1000.00

     

    NWHS is currently seeking a compassionate and proactive Nurse Visit RN/LPN to join our Patient-Centered Medical Home (PCMH) team, where you'll support high-quality, nursing care visits that promote prevention, chronic condition management, and care gap closure.

    YOUR ROLE:

    As our Nurse Visit RN/LPN, you’ll work side-by-side with providers, behavioral health professionals, medical assistants, and care coordinators to empower patients through education, access, and coordinated services that improve health outcomes.

    SPECIFIC DUTIES:

    + Participate in nurse visits for services such as immunizations, wound care, diabetes support, and contraceptive injections under the clinical direction of an RN or licensed provider.

    + Collaborates with care team to ensure seamless and patient-centered care.

    + Reinforce patient education related to prescribed medications, chronic condition management, and proper use of medical devices, as directed by the care plan.

    + Perform focused assessments and collect clinical data (e.g., vital signs, symptoms, patient history) to support the RN or provider’s evaluation.)

    + Ability to recognizes abnormal and critical clinical findings and following established protocols and take appropriate action including notifying provider as appropriate.

    QUALIFICATIONS:

    + Current Oregon RN/LPN license.

    + Minimum of 1-2 years of clinical nursing experience, preferably in an ambulatory care, community health, or FQHC setting.

    + Has strong clinical assessment, problem-solving, and decision-making skills with sound clinical judgement.

    + Able to provide compassionate, culturally competent care delivery.

    + Current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, and covers breathing and cardiac emergencies - including CPR, AED, and obstructed airway - for adult, child, and infant patients.

    + Ability to perform focused assessments and contribute to care planning

    + Bilingual preferred but not required.
