Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Responsibilities The incumbent in this position represents a Community Care Program Supervisor in this medical center. The incumbent supervises all pharmacy related activities of VA authorized community care programs regarding medication order review and processing. The incumbent plans and coordinates pharmacy services for the VA community care program patients and providers within the guidelines of the program, Joint Commission standards, and medical center policies/regulations. The incumbent must be able to demonstrate knowledge of current pharmacy laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. The incumbent is responsible for addressing a variety of questions posed by physicians, nurses, coworkers, and patients related to drugs, pharmacy services and departmental policies and procedures. The incumbent must be able to interpret and apply VA, federal, and state regulations, Joint Commission, DEA, FDA, hospital, and Pharmacy Service polices under all circumstances. Duties and responsibilities include but are not limited to: Program Supervision - VA Community Care Programs Provides supervisory pharmacist oversight for VA Community Care Programs by reviewing and entering outside prescriptions written by VA Community Care Programs providers for patients enrolled at the medical center. Serves as consultant and information resource to the VA Community Care Programs physicians and applicable medical center staff and patient care services. Ensures patient prescriptions written by outside providers conform to both national and local VA formulary and adjudicates non-formulary and restricted requests if necessary. Performs supervisory functions of staff assigned to the pharmacy Community Care team including performance reviews, leave approval and training requirement. The incumbent will ensure that subordinates follow VA, VISN, and medical center directives and guidelines during the performance of their duties. Program Supervision - Therapeutic Interchange The incumbent provides clinical leadership for medical center patients, caregivers, and providers when therapeutic interchanges are mandated / deemed prudent. This may be achieved either verbally or via written correspondence. Assists in formulary management through facilitation of therapeutic substitutions that do not require a Scope of Practice, non-formulary reviews, medication use evaluations, and documenting/assessing adverse drug events. Documents and reports drug cost savings relative to therapeutic interchange to the Associate Chief of Operations. Pharmacy Service Involved in reviewing patient medication regiment for cost effective drug. Reviews, interprets, and verifies medication orders for appropriateness, processes medication orders, and interacts/makes recommendations to other clinical staff regarding medication therapy ordered to ensure safe and cost-efficient effective care. Reviews patient medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems, contacting providers as appropriate. Documents recommendations and interventions. Provides refill extensions and partial medication supplies when appropriate. Substitutes alternate dosage strength or products per facility policy. Interprets health and medication histories, performs medication reconciliation, and provides drug information. Provides written and verbal (telephonic) patient education to patients. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grade Determinations: S-13 Pharmacy Supervisor Experience and Education: In addition to the basic requirements listed above, you must have 1 year of creditable experience equivalent to the next lower grade level (GS-12) and have completed of an ACPE-accredited Pharm.D. program. Pharmacy Supervisor. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. Preferred Experience: Previous leadership experience a plus. Experience with receiving, reviewing, and processing prescriptions from VA-approved community care providers.
