Position Overview: Under the supervision of the Director of Centralized Intake and Managed Care Relations, the Intake & Managed Care Coordinator oversees all referrals and intake operations for Day Treatment and Aftercare programs as well as assists with referrals and intake operations for the Residential program. This position also maintains a marketing component by promoting these programs to referral sources and other stakeholders throughout the Capital Region and New York State.
Position Requirements & Essential Attributes:
+ + Bachelor’s Degree in social work, psychology, or human services, with a minimum of 2 years of human services experience.
+ Experience working with child-welfare involved children and families, preferred.
+ Excellent organization, planning and time management skills.
+ Previous project management experience, preferred.
+ Strong interpersonal skills with the ability to build strong working relationships with regulatory bodies.
+ Clean & Valid New York State Driver’s License (actively licensed for at least one year) with a reliable vehicle for transporting clients as needed, is required*.
+ Flexibility to work hours outside of regularly scheduled work hours, when needed.
+ Ability to learn and operationalize the principals of trauma-informed care as they relate to the intake process and foster care operations.
+ Excellent organizational, planning, and time management skills.
+ Strong interpersonal skills and empathy for those in need.
+ Outstanding communication skills, with the ability to build strong, collegial working relationships with referring organizations and regulatory bodies.
+ Proficiency in various computer applications such as Microsoft Office Suite, Google Business Suite, and use secure Internet practices.
+ Experience with AWARDS is a plus!
+ Ability to function well in a high-paced and, at times, stressful environment.
+ Ability to handle sensitive data with strict confidentiality. [The ability to process and actively safeguard client protected health information while maintaining strict client confidentiality is essential to this position.]
+ Self-motivated and engaged, with a strong attention to detail.
+ Ability to understand, represent, and carry out the mission, values, philosophy, and policies of St. Catherine’s Center for Children.

+ Ability to pass required pre-employment background checks.
*This position is a driving position that must be able to meet the minimum requirements of the agency’s insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire.
Essential Duties & Responsibilities include:
+ + Receives referrals, provides immediate feedback to referral sources, and coordinates and conducts intake and admissions operations specific to Residential and Day Treatment programs. Will forward all CFTSS referrals to the designated CFTSS staff.
+ Conducts intakes in a manner consistent with the values and principles of Trauma Informed Care and Child and Adolescent Service System Programs (CASSP).
+ Ensures that the goals and methods of treatment reflect the choices of the consumer as well as the priorities of the referring bodies.
+ Ensures that the admission decision-making process is consensus driven by respecting the observations and input of the entire team including the child, family and referring agency.
+ Maintain a working knowledge of the admission and eligibility criteria of all of the programs in the Centralized Intake continuum. This includes understanding all NYS regulatory and MCO guidelines pertaining to intake and admission.
+ Works with the Director of Centralized Intake and Managed Care Relations to insure all admission requirements set by the child’s managed care plan are met. This includes verifying eligibility and ensuring that medical necessity for the level of care is met.
+ Works closely with Program Management Teams in order to assist those teams in planning for and projecting census into the next fiscal year.
+ Cultivates a vision of a "Continuum of Care" to ensure that the client is admitted into the program that will provide the least restrictive level of care based on the corresponding level of need.
+ Responds to emergency referrals as soon as possible, but no later than the next business day.
+ Ensures that all applicable documentation for each referral and intake meeting is completed, and all appropriate staff have been provided with the documentation to prepare for the child’s admission.
+ Chairs weekly Placement Meeting to review referrals, admissions and discharges to Residence and Day Treatment and sends timely meeting minutes afterward.
+ Chairs all intake and attends emergency placement meetings, when necessary.
+ Promotes and establishes community contacts to raise awareness of St. Catherine’s programs.
+ Uses AWARDS database to ensure data is collected and consistent with agency and regulatory bodies reporting requirements.
+ Completes the intake portion of the Intake/Assessment form and obtains required consent forms.
+ Collects, organizes and reports on referral and intake data as needed for annual reports and to comply with regulatory mandates.
+ Assists in tracking the credentialing of licensed professional staff to ensure ongoing compliance of licensing requirements.
+ Assists families and counties in re-certifying clients with Medicaid and Managed Care Plans as needed.
+ Additional position-related duties and responsibilities as assigned by the Director of Centralized Intake and Managed Care Relations.
