Job Overview
Responsibilities
The **Project Coordinator** for the Hepatitis C Grant is responsible for day-to-day management and operational coordination of the Hepatitis C Elimination Initiative Pilot at Hillcrest Family Services. This position plays a key leadership role in implementing integrated behavioral health and infectious disease services – focusing on same-day HCV/HIV testing, care coordination, and linkage to treatment for individuals with or at risk of Hepatitis C, Substance Use Disorder (SUD), Serious Mental Illness (SMI), and homelessness. Working closely with the Project Director and Nurse Care Manager, the Project Coordinator ensures effective service delivery, staff supervision, compliance with SAMHSA requirements, data reporting, and partnership engagement to achieve the program’s goal of eliminating HCV among high-risk populations. Additional responsibilities include:
+ OverseedailyoperationsoftheHCVEliminationPilotProgramandsuperviseassignedstaff.
+ EnsuretimelyimplementationofrequiredactivitiesunderSAMHSA’sfiveservicedomains:OutreachandPrevention, Same-Day Screening, Testing and Treatment Initiation, Ongoing Treatment, Peer Support and Care Coordination, and Evaluation and Reporting.
+ Coordinateprogramworkflowstosupportlow-barrieraccesstointegratedbehavioralhealth,SUD,andinfectious disease services.
+ MaintaincompliancewithSAMHSAguidelines,datacollectionstandards,andExecutiveOrders.
+ Buildandsustainpartnershipswithcommunityorganizations,publichealthdepartments,medicalproviders,housing agencies, and Ryan White HIV/AIDS Program (RWHAP) partners.
+ Supportoutreachandpreventioncampaignsfocusedonreducingstigma,increasingawarenessofHCVandSUB, and promoting harm reduction strategies.
+ Coordinatecommunity-basedevents,mobileoutreach,andeducationaleffortstargetingat-riskpopulations.
+ CollaboratewiththeNurseCareManagerandPeerSupportstafftoensureclientsreceivethesame-dayscreening, facilitated referrals, and follow-up care.
+ Supportlinkagetohousing,transportation,vocational,andbenefitservicesaspartofholisticcarecoordination.
+ Ensureaccurateandtimelydocumentationofclientencounters,testresults,andfollow-upoutcomes.
+ MonitorprogrammetricsandoutcomedatausingSAMHSA’sSPARSandorganizationalEHRsystems.
+ AssistwithquarterlyperformancereportingandannualprogrammaticprogressreportstoSAMHSA.
+ ParticipateinSAMHSA’scross-siteevaluationandlearningcollaborativestosharebestpracticesandprogram insights.
+ Develop,implement,andupdateoperationalprocedurestoensurecompliancewithfederalandstatestandards.
+ Maintainaccurateprogramdocumentation,includingconsentforms,MOUs,datacollectiontools,andperformance measures.
+ Assistinbudgettrackingandreporting,ensuringefficientandcompliantuseoffederalgrantfunds.
+ UpholdconfidentialitystandardsinaccordancewithHIPAAandorganizationalpolicies.
+ Otherdutiesasassigned.
Requirements
Education
+ Bachelor’sdegreeinPublicHealth,SocialWork,Nursing,Psychology,Healthcare Administration,orrelatedfieldrequired.
+ Master’sdegreepreferred.
Experience:
+ Minimumof3years’experienceinprogramcoordination,healthcareoperations,or behavioral health.
+ PriorexperiencewithSAMHSA-fundedorHRSA/RyanWhiteprogramspreferred.
+ Experienceincoalitionbuildingandcommunityoutreachpreferred.
Knowledge/Skills:
+ DemonstratedknowledgeofHCV,HIV,SUD,and/ormentalhealthservicesand community-based healthcare models.
+ Experiencewithfederalgrantadministration,datacollection,andperformance reporting preferred.
+ Strongskillsincollaboration,communication,problem-solving,andproject management.
+ ProficiencyinMicrosoftOfficeSuite,EHRsystems,anddatabasemanagement.
+ Commitmenttoharmreduction,trauma-informedcare,andequitableaccesstohealth services.
+ FamiliaritywithSPARSdatareportingorsimilarperformancetrackingsystems preferred.
Licenses/Certifications:
+ MandatoryReporterofChild/DependentAdultAbuseCertificationortheabilityto obtain within six (6) months of hire and every three (3) years thereafter required.
