Job Summary and Qualifications
POSITION SUMMARY:
Responsibility includes the provision of clinical services and the coordination of care for transplant and living donor patients. Provides education and clinical support for staff in the Transplant Center and in the hospital. Works cooperatively and effectively as a member of the Multidisciplinary Care team.
POPULATION SERVED:
All employees, staff of LPMC in both clinical and non-clinical departments, including patient/families, physicians, volunteers, agency, contract, students and the community served.
ESSENTIAL FUNCTIONS:
Evaluates and monitors all patients under consideration for kidney transplantation, Wait Listed, or living donation per established protocols, evaluates outcomes, and develops a plan of care.
+ Provides education to recipients and donors and their families regarding pre and post-transplant nutrition.
+ Evaluates, monitors, and educates post-transplant patients and their families on post-transplant nutrition.
+ Must understand the disease process as well as dialysis needs for patients undergoing transplant evaluation and follow up.
+ Must determine recommendations for nutritional status improvements and provide direction and instruction for resolution of barriers to transplantation.
+ Follows all regulatory requirements to ensure program viability.
+ Attends Grand Rounds
+ Assesses and educates newly transplanted patients in the inpatient setting and documents according to protocol.
+ Documents appropriately in the patients’ medical record all interventions and recommendations to determine candidacy for kidney transplant.
+ Collaborates with medical team as well as referring dieticians to determine the best kidney transplant plan of care and waitlist nutritional management.
+ Demonstrates professionalism, teamwork, and initiative in working with others while maintaining the ability to self-schedule for dietary evaluations.
+ Supports the mission and vision of the Las Palmas Medical Center and demonstrates excellent customer service at all times.
+ Understands and committed to helping the Las Palmas Medical Center and its associated facilities achieve its goals.
+ Must lean and comply with Las Palmas Medical Center facility safety policies and rules; must use appropriate safety equipment and procedures at all times; must immediately report all unsafe conditions to Transplant Clinic Director; must be familiar with all safety features of equipment, tools or materials encompassed by job duties; and must check with Transplant Clinic Director (prior to job performance) if there is a question as to safe procedure to be used for any job function.
+ Adheres to the LPMC Code of Conduct and demonstrates ethical behavior as expected by our patients, colleagues and community. Understands the personal obligation to report any activity that appears to violate applicable laws, rules regulations or Code of Conduct itself.
+ Documents findings accordingly.
+ Participate in In-Services for patient education.
+ Participate in staff education.
