    What We Offer

     

    Support Specialist - Home Care Team

     

    Work Schedule: Monday - Friday from 8 am - 5 pm.

     

    Rotation: Holidays rotate coverage and a total of 3 Support Specialists.

     

    Optional: At night phones roll to on-call. Option for additional pay if they elect to cover night phone @$30/night, regardless if there are no calls.

     

    Status: Non-Exempt/Hourly

     

    Responsible for routine clerical duties related to patient care delivery, including phone responsibility and assuring medical documentation and patient scheduling flows in a coordinated and efficient manner.

    Responsibilities

    + The team member’s primary responsibility is to deliver the most remarkable patient experience in every interaction, understanding how their role contributes to the health system’s vision of excellence in patient and family care.

    + Responsible for routine clerical duties related to patient care delivery, including phone responsibility and assuring medical documentation and patient scheduling flows in a coordinated and efficient manner.

    + Maintain scheduling for RN/Case Managers and triaging patients by phone, based on script provided.

    + Create a strong rapport with field RNs and patients.

    + Excellent customer experience with patients and staff.

    + Excellent multi-tasking and troubleshooting. Work side by side with Clinical Supervisors.

    + Responsible for patient/deficiency/account work queues.

    + Updating accounts with accurate discharge information for accurate billing.

    + Detail oriented and problem solver. Also engagement coordinator to support Director with engagement activities for the department.

    + Create/manage Excel spreadsheets.

    Qualifications

    + Required Education: High School Diploma or GED.

    + Required Experience: 2 years experience in a clinical or healthcare setting. A highly organized individual who enjoys multi-tasking and supporting a team. Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence. Previous health care administrative support experience. Previous Excel spreadsheet experience.

    + Preferred Education: 2 Year / Associate Degree. Two year degree preferred in health or business related field.
