Patient Care Coordinator | November Start – Palo Alto

Start Date: November, 2025 | 13-Week Contract

We’re seeking an experienced Patient Care Coordinator (PCC) for a local contract at a leading healthcare facility in Palo Alto, California. This role supports outpatient operations and care coordination, working closely with clinical teams to ensure smooth patient experiences.

Position Highlights

+ Location: Palo Alto, CA

+ Setting: Outpatient Clinic

+ Start Window: Early November

+ Contract Length: 13 weeks, with potential for extension

+ Schedule: 5x8s (40 hours/week)

+ Shift: Days

Required Qualifications

+ CMA certification

+ BLS certification

+ 3+ years of PCC or Medical Assistant experience

+ EPIC EMR proficiency

Additional Compliance Requirements

+ TB Questionnaire, PPD, or chest X-ray (if applicable)

+ Current Health Certificate (per contract or state regulation)

+ Must meet all federal, state, and local requirements

+ Successful completion of new hire training as applicable to job site

+ Understanding of patient confidentiality and HIPAA requirements

+ Ability to effectively communicate with supervisors, co-workers, and clients; proficiency in English required

+ Computer proficiency required

What You’ll Do:

Coordinate patient care activities including scheduling, documentation, and communication between departments. Support clinical staff and ensure patients receive timely, organized, and compassionate care.
