Department: Care Navigation - supporting heart failure patients

This role is ideal for an experienced RN looking to serve as the primary liaison between the patient and his/her medical team and be a patient advocate.

Serves as primary liaison between the patient and his/her medical team as well as patient advocate and support.

Responsibilities

+ Support heart failure patients on any unit at HMC

+ Find resources for patients including food, medicine, and clinics

+ Work to discover what is causing the patient to return to the hospital

+ Review medication and diet adherance issues with the patient

+ Provide some educaiton on the patient's diagnosis

+ Partner with case management and palliative care teams

+ Work towards goal of reducing readmission and ER visits

Qualifications

+ Education: 4 Year / Bachelors Degree, required.

+ Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.

+ Licensure/Certification: RN, required.

+ Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
