    Cambridge Health Alliance's Program of All-Inclusive Care for the Elderly (PACE) is a nationally recognized model of care that serves adults aged 55 and older living with chronic medical and/or mental health challenges.

     

    At CHA PACE, our mission is to support our program participants in achieving the highest level of independence within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment.

    Position Overview

    As a Behavioral Health Clinician in the PACE program, you will:

    • Provide a wide range of clinical psychosocial services both at our PACE center and in participants' homes and community settings.

    • Conduct initial, semi-annual, and annual psychosocial assessments.

    • Deliver clinic-based and in-home clinical case management, supportive counseling, crisis intervention, and resource coordination.

    • Collaborate with the Interdisciplinary Team to carry out individualized care plans tailored to each participant’s needs.

    • Offer support to family and collateral contacts, including conducting visits in participants’ residences and care facilities.

    Qualifications

    • Licensure: Current Massachusetts LMHC, LCSW, or LISCW license in good standing

    • Work Experience: Minimum 1 year experience providing clinical social work to older adult population preferably in a community or medical setting

    • Proficiency with therapeutic modalities such as Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, or Mindfulness Based therapy

    • Skilled in clinical documentation; experience with EPIC beneficial

    • Reliable transportation for home and community visits

    • Ability to work collaboratively in a variety of community settings, including Assisted Livings, Nursing Homes, and Community Housing

    • Experience working with individuals with dementia and their families and support networks beneficial

    • Commitment to equitable and culturally appropriate care for a wide range of diverse populations, including (but not limited to) communities of color, LGBTQ communities, non-English speaking populations, people with histories of incarceration, people with substance use disorders and behavioral health challenges

    • Fluency in Spanish, Haitian Creole or Portuguese a plus

    • Interest in working in a collaborative, team environment
