    **Pay Range:** $53.73 - $84.63 per hour

    Summary:

    The Complex Outpatient Antimicrobial Therapy (COpAT) Pharmacist is a clinical pharmacy specialist and leader responsible for the development, implementation, and documentation of the outpatient antimicrobial therapy program for the Rush University Health System (Rush). This includes providing multidisciplinary education on safe antimicrobial use, serving as a resource for infectious diseases providers in the outpatient setting, collaborating with multidisciplinary clinicians, administrators, and support staff across the system, and analyzing metrics for antimicrobial utilization to satisfy new and existing CDC and Joint Commission standards. The COpAT pharmacist will work in congruence with the system antimicrobials stewardship pharmacist (ASP) team and responsible for developing and coordinating innovative clinical pharmacy programs across Rush sites conducting scholarly and research activities and interdisciplinary teaching. To support the aims of the program, the COpAT pharmacist will maintain on-site coverage in the infectious diseases clinic. The COpAT pharmacist will serve as a key preceptor in postgraduate pharmacy training programs and affiliated colleges of pharmacy.

    Job Responsibilities:

    + Optimize and monitor safe and effective antimicrobial therapy through therapeutic drug monitoring, lab review, and timely dose adjustments while ensuring safe transitions between IV and oral regimens.

    + Serve as a resource to infectious disease providers for drug information.

    + Serve as a key liaison between patients and the multidisciplinary team, supporting individualized treatment plans and continuity of care throughout outpatient therap.

    + Develops metrics for performance and maintenance of ASP.

    + Coordinate with Rush specialty pharmacy and home infusion pharmacy for medication access support and coordination.

    + Participate in ASP coordinated education of healthcare professionals across Rush sites and external partners.

    + Serves as a liaison to other healthcare providers in the ASP.

    + Coordinate the implementation of identified ASP initiatives.

    + Coordinate with Rush clinics for various clinical and quality improvement projects.

    + Assist with regulatory and accreditation requirements per CMS/The Joint Commission.

    + Assists with the strategic implementation of Department of Pharmacy and Rush quality initiatives.

    + Serve as a preceptor for postgraduate year 1 and 2 (PGY1 and PGY2) pharmacy residents and students and infectious diseases medical fellows.

    + Document clinical interventions in EPIC.

    + Participate in the Antimicrobial Stewardship Subcommittee of the System Pharmacy and Therapeutics Committee.

    + Complete medication use evaluations (MUE) and new drug and therapeutic class reviews for System P&T as assigned.

    + Provide pharmacy representation in multidisciplinary committees, when appropriate.

    + Additional Clinical/Operational Responsibilities include:

    + Evaluate, clarify, and verify EPIC orders for therapeutic appropriateness, when necessary.

    + Serve as content expert for ID related drug shortages.

    + Participate in safety event (SE) reporting.

    + Facilitate patient medication reconciliation and teaching when requested.

    + Maintain working knowledge of medical center and departmental policies and procedures.

    Required Job Qualifications:

    + Bachelor of Science in Pharmacy.

    + Illinois Pharmacist License.

    + Minimum of four years of Clinical Pharmacy experience OR PGY 1 residency completed within the last seven years, which includes Clinical / Specialty Pharmacy experience.

    + Knowledge in intravenous admixture services, intravenous drug therapy and chemotherapy.

    + Must be able to work effectively with others to provide safe and effective drug therapy.

    + Excellent problem-solving skills and the ability to work independently and quickly in responding to and resolving a wide range of problems.

    Preferred Job Qualifications:

    + Doctor of Pharmacy degree from an accredited College of Pharmacy.

    + Completion of a PGY2 pharmacy residency in Infectious Disease and/or an Infectious Disease focused Fellowship.

    + Board Certification in Infectious Diseases Pharmacotherapy prior to or within 18 months of hire.

    + Demonstrated ability to work effectively with multidisciplinary stakeholders, strong clinical acumen and understanding of medical literature, and ability to drive large-scale projects forward.
