Description of Job
The Work Unit:
The Office of STI/HIV/VH works to reduce disease transmission and ensure quality and medically accurate STI/HIV related prevention, care, and treatment services for all people in Colorado. Functions include public health surveillance of STI/HIV/VH; follow-up of people newly diagnosed with a STI/HIV to ensure adequate treatment; referrals and partner notification; access to care and treatment as appropriate; developing comprehensive prevention programs; and conducting program evaluation to enhance the use of STI/HIV/VH data to direct prevention and care activities. The Care and Services Program exists within the Office of STI/HIV/VH to provide access to health care, support for clients and linkage to care, and intensive case management among clients with needs related to HIV and STI. The Office of STI/HIV/VH, operates on the belief that all people - no matter their race, ethnicity, gender identity, sexual orientation, education level, age, language, religion, ability or geographic location - should have the opportunity to live healthy lives. The Program has three units: Health Care Access, Linkage to Care, and Client Support. The Health Care Access unit exists to ensure low-income individuals, who are either living with HIV or are at Risk of acquiring HIV, have access to HIV and STI medications, medical care and treatment.
What You Will Be Doing:
This position assists clients in navigating considerations involved in receiving health care coverage and access to complex treatment regimens. This position determines and assesses SDAP eligibility and eligibility for subprograms within SDAP, as well as facilitates enrollment in healthcare coverage according to need.
Responsibilities of the position include but not limited to:
Eligibility Determination
+ Directly provides services according to the established Colorado Ryan White standards of care assessments/reassessments, service plans, coordination, monitoring of medical treatment, education, patient self-management etc.
+ Anticipates and analyzes the consistency of engagement in care for persons living with HIV and provides referrals to all other appropriate wrap-around services and programs available throughout Colorado and the community to address any identified barriers or challenges.
+ Facilitates enrollment into the correct SDAP subgroup to ensure the program is compliant with payor of last resort requirement.
+ Conducts the initial certification and annual recertification processes for people enrolled in SDAP.
+ Assesses the declarations made by clients on forms, and the accompanying documentation in order to determine if clients meet residency and income requirements as set by the Program and Unit.
+ Utilizes the unit's designated data systems to document client interactions and eligibility determinations completely, accurately and in a timely manner, consistent with established performance standards.
+ Document any interaction with the client in notes.
+ Detect erroneous, false, or fraudulent information provided by clients, and acts to prevent harm to the program x Ensure that documentation of client residence, income, and payer of last resort meet federal and state expectations.
+ Collecting and recording accurate, complete client data in a timely manner.
Health Care Navigation
+ Provides Health Care Navigation for persons living with HIV and those vulnerable to acquisition.
+ Analyzes information provided by clients and facilitates enrollment in healthcare coverage.
+ Facilitates access to the State Drug Assistance Programs (SDAP).
+ Reviews application and arranges for rapid access to health care coverage and prevents lapses in care.
+ Conducts a comprehensive assessment of the client’s health care needs including both Medication Assistance and Insurance Assistance.
+ Provides health education regarding coverage eligibility and what best meets the client's needs.
+ Facilitates enrollment in healthcare coverage.
+ Identify clients whose care has lapsed or who are in transition and provide referrals to case management, medical care, and other services.
+ Refers clients to a broad spectrum of services that improve the success of staying engaged in care.
+ Follow up with clients regarding their Advanced Premium Tax Credit (APTC) and vigorously pursue any amount owed back to the program.
+ Assist in the completion of a cost analysis on the alternative plan requests to ensure the program is complying with the “Payor Last Resort” requirement.
Capacity Building and Technical Assistance
+ Provides capacity building and technical assistance to assist providers and case managers in the implementation of the State Drug Assistance Program.
+ Troubleshoots data system problems and inefficiencies and maintains safety and confidentiality of all HIV related public health documents.
+ Participates in quality improvement activities for the Health Care Access Unit.
+ Research clients' records to troubleshoot any issues preventing claims from being adjudicated at the pharmacy and relay that information to the client or provider to ensure claims are paid for at point of sale.
+ Provide technical assistance to clients to help enroll into coenroll.com, the SDAP online portal.
+ Assist clients in uploading and submitting any documentation requested to prove eligibility.
+ Complete data entry audits to ensure accuracy of data.
Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Minimum Qualifications:
NOTE: An applicant may meet the minimum qualifications for this job in one of two ways: They may qualify through their work experience alone or through a combination of education and work experience.
Experience Only:
Five (5) years of relevant experience in providing health navigation, insurance navigation and/or enrollment , public health or healthcare administration, providing services to clients in the STI/HIV or another public health field, STI/HIV testing, prevention services, health services street outreach and/or other one-on-one health education, community engagement, analyzing and reporting data, troubleshooting data systems, or other closely related experience in an occupation related to the work assigned to this position.
OR:
Education and Experience:
A combination of related education from an accredited institution in Public Health, Health Education, Health Care Administration, or a field of study closely related to the work assignment. AND/OR Relevant experience in health navigation, insurance navigation and/or enrollment, public health or healthcare administration, providing services to clients in the STI/HIV or another public health field, STI/HIV testing, prevention services, health services street outreach and/or other one-on-one health education, community engagement, or other closely related experience in an occupation related to the work assigned to this position equal to five (5) years.
