UPMC Western Maryland is seeking a full-time LPN to support our Home Health department!

This is a Monday-Friday, dayshift position that includes a holiday coverage rotation. As the LPN, you will provide direct technical nursing care to patients/families within their home setting under the direction of the primary care nurse and in conjunction with the interdisciplinary team.

Responsibilities:

+ Maintains a confidential record of patients' progress, symptoms, reactions, treatments performed, interventions, and care provided. Completes documentation, including visit assessment and verbal orders, accurately and promptly on the laptop device or in the written documentation while in the patient's home and within the agency and regulatory standards. Data syncs (transmits) information the same day. This information is communicated to the primary service provider and appropriate interdisciplinary team members in a timely and thorough manner.

+ Provides individualized skilled home health nursing care to patients under the supervision of a registered professional nurse.

+ Provides individualized patient/caregiver teaching and appropriate counseling related to health prevention, health maintenance, and disease process under the direction of the registered nurse.

+ Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

+ Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.

+ Maintains nursing knowledge base through in-services, literature review, mandatory, and competency programs.

+ Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC/Jefferson Regional Home Health, L.P. towards patients, visitors, staff, peers, physicians, and other departments within the organization.

+ Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures.

+ Assists the registered nurse/therapist in assessment of the patient's condition and contributes information to aid in planning and evaluating care. Considers the physical, psychosocial, and emotional needs of the patient.

+ Graduate of a Practical Nursing Program approved by the State Board of Nursing is required.

+ One (1) year of Medical/Surgical experience in an acute care facility and/or skilled nursing facility required. Minimum of 2 years med/surg experience in an acute care and/or skill nursing facility preferred.Licensure, Certifications, and Clearances:

+ Automotive Insurance

+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) that include both a didactic and skills demonstration component within 30 days of hire.

+ Driver's License

+ Licensed Practical Nurse (LPN)

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
